Did you know that dementia affects one out of three dying older adult Americans? Compassion & Choices’ Dementia Values & Priorities Tool is a free online resource that allows individuals to give their healthcare proxies and healthcare providers detailed information about their preferences at various stages of dementia progression, taking the guesswork out and helping to ensure the person’s wishes are fulfilled.

We will also be showcasing a video on dementia, which tells the story of dementia from the perspective of the people impacted by it. We hope you’re able to join us for this unique educational event.
